Terms:

• Vaccination boosters will not be included within the plan if due within the first 3 months after the start date. If they are due within the first 3 months, they will be charged for at the standard fee.

• Direct debit will be taken at the 3rd of each month. If an alternative date has been selected when joining online, the date will be altered to the 3rd of the month.

• It is the clients responsibility to book their own appointments and collect their parasite treatments.

• Any uncollected treatments at the end of each 12 month plan period can not be refunded.

• If a Direct Debit is cancelled, then the vlaue of treatments collected will be calculated and will be owed to the Practice.

• If a Direct Debit collection fails, then the client is liable for a £10 bank charge.

• A client has the right to cancel at any time, but must do so by contacting the surgery.
